We are looking for nurses who can commit to working the following hours- Monday to Friday, 08:00 - 16:00.
Main duties-
- Providing community rehabilitation to patients within Birmingham
- Ordering equipment and reviewing this where needed
- Completing holistic health assessments and care plans.
- Preparing and monitoring treatment plans in line with best practice and evidence.
- Taking part in team in-service training and contributing where possible.
- Referring to services within the team and within the locally community.
- Attending MDT and team meetings and feeding back to colleagues.
- Must be car driver.
