Job Description
As a Community Mental Health Nurse, you will work closely with service users, carers, and healthcare professionals to deliver effective mental health interventions. This is a fully site-based role requiring strong clinical skills, sound professional judgement, and excellent communication.
Key Responsibilities:
Deliver high-quality mental health nursing care to service users in the community
Undertake mental health assessments, care planning, and risk assessments
Manage and support a caseload in collaboration with the wider MDT
Provide therapeutic interventions and ongoing clinical support
Monitor patient progress and adapt care plans as required
Maintain accurate and timely clinical documentation
Liaise with external agencies, carers, and families to ensure holistic care
Requirements:
Registered Mental Health Nurse (RMN) with active NMC registration
Experience at Band 5 or Band 6 level
Previous community mental health experience (desirable)
Strong assessment, communication, and clinical decision-making skills
Ability to work effectively as part of a multidisciplinary team
Availability to work fully on-site (hybrid working not available)
