Job Description

As a Community Mental Health Nurse, you will work closely with service users, carers, and healthcare professionals to deliver effective mental health interventions. This is a fully site-based role requiring strong clinical skills, sound professional judgement, and excellent communication.

Key Responsibilities:

  • Deliver high-quality mental health nursing care to service users in the community

  • Undertake mental health assessments, care planning, and risk assessments

  • Manage and support a caseload in collaboration with the wider MDT

  • Provide therapeutic interventions and ongoing clinical support

  • Monitor patient progress and adapt care plans as required

  • Maintain accurate and timely clinical documentation

  • Liaise with external agencies, carers, and families to ensure holistic care

Requirements:

  • Registered Mental Health Nurse (RMN) with active NMC registration

  • Experience at Band 5 or Band 6 level

  • Previous community mental health experience (desirable)

  • Strong assessment, communication, and clinical decision-making skills

  • Ability to work effectively as part of a multidisciplinary team

  • Availability to work fully on-site (hybrid working not available)
