As a Community Nurse, you will provide holistic nursing care within the community, visiting patients at home to complete assessments, deliver treatments, and support ongoing care needs. Applicants must have proven community experience.
Key Responsibilities:

• Provide nursing care in patients’ homes

• Wound care, medication administration, injections, observations

• Conduct assessments and develop care plans

• Work with MDTs for coordinated care

• Maintain accurate clinical documentation

• Support patients and families with health education

Requirements:

• Valid NMC registration

• Proven community nursing experience

• Ability to work independently

• Strong organisational and communication skills

• Right to work in the UK
