The successful candidate will provide nursing care to patients within the community, supporting their health and wellbeing while promoting independence and dignity. You will be expected to travel between locations and adapt to varying patient needs.

Key Responsibilities:

• Provide holistic nursing care within community settings

• Assess, monitor, and review patient conditions

• Implement and review care plans in line with best practice

• Administer prescribed treatments and medications

• Liaise with GPs, district nurses, and wider MDTs

• Ensure accurate documentation and compliance with policies

Requirements:

• NMC registered Nurse

• Previous community nursing experience (essential)

• Own car and full driving capability (essential)

• Excellent communication and interpersonal skills

• Flexible and reliable approach to work
