In this role, you will support patients with a range of healthcare needs, helping to prevent hospital admissions and promote recovery at home. You will work independently while being supported by a wider healthcare team.

Key Responsibilities:

• Deliver patient-focused nursing care in the community

• Carry out clinical assessments and ongoing patient reviews

• Administer treatments and medications safely

• Support patients and families with education and advice

• Maintain accurate patient documentation

• Travel between patient visits using your own vehicle

Requirements:

• Registered Nurse with active NMC registration

• Demonstrable community nursing experience

• Access to your own car (essential)

• Strong clinical judgement and decision-making skills

• Ability to work independently and manage workload effectively
