As a Community Psychiatric Nurse, you will play a key role in supporting adults with complex mental health needs, promoting recovery, independence, and wellbeing within the community.
Key Responsibilities:
Manage a defined caseload of service users with complex and enduring mental health needs
Undertake comprehensive mental health and risk assessments, developing and reviewing person-centred care plans
Deliver evidence-based nursing interventions, including medication management and psychoeducation
Act as Care Coordinator under the Care Programme Approach (CPA) where appropriate
Work collaboratively with psychiatrists, psychologists, social workers, GPs, and voluntary sector partners
Monitor mental state, identify early warning signs, and support relapse prevention and recovery goals
Maintain accurate, timely, and high-quality clinical records in line with professional and organisational standards
Safeguard vulnerable adults and contribute to effective risk management planning
Participate in multidisciplinary team meetings, clinical supervision, and ongoing service development initiatives
Person Specification
Essential Criteria:
Registered Mental Health Nurse (RMN) with current NMC registration
Experience working within mental health services (community and/or inpatient)
Sound knowledge of the Mental Health Act, Mental Capacity Act, and Care Programme Approach (CPA)
Strong assessment, communication, and organisational skills
Ability to work autonomously while contributing effectively within a multidisciplinary team
Full UK driving licence or ability to travel across the borough
Desirable Criteria:
Previous experience working within a CMHT or community mental health setting
Non-medical prescribing qualification
Experience supporting individuals with severe and enduring mental illness (SMI)
Knowledge of recovery-oriented and trauma-informed care approaches
Relevant post-registration training in mental health nursing
