Clinical Duties
Manage a personal caseload, including assessment, planning and delivering evidence-based mental health interventions.
Provide support across the stepped care model, including wellbeing and primary mental health interventions.
Work collaboratively with primary care, substance misuse teams and prison staff.
Arrange referrals, aftercare, and coordinated support with external services.
Maintain high standards of documentation, confidentiality, and compliance with relevant legislation including the Mental Health Act.
Deliver safe, compassionate care to individuals with a range of complex mental health needs.
Liaison & Multi-Agency Working
Participate in MDT meetings, case reviews and MAPPA discussions.
Build strong working relationships with healthcare, prison, statutory and voluntary partners.
Promote continuity of care and accessibility of mental health services.
Leadership & Professional Standards
Support service improvement and assist the Team Manager with clinical objectives.
Provide guidance, leadership and supervision to junior staff where required.
Demonstrate excellent time management, prioritisation and professional boundaries.
Support incident reporting, risk management and quality governance processes.
Communication & Governance
Maintain accurate, timely electronic records in line with NMC and NHS standards.
Communicate effectively with patients, families, colleagues and partner agencies.
Contribute to audits, quality improvement and adherence to CQC standards.
Risk Assessment & Safeguarding
Conduct thorough risk assessments and implement appropriate risk management plans.
Recognise and respond to crisis situations, safeguarding concerns and vulnerabilities.
Work within health and safety requirements to maintain a safe environment for all.
