As a Community Nurse, you will be responsible for providing safe, effective, and compassionate nursing care to patients within the community. You will work closely with multidisciplinary teams to support patient wellbeing, promote independence, and reduce hospital admissions.
Key Responsibilities:
Delivering nursing care to patients in community and home-based settings
Conducting patient assessments and care planning
Administering medications and treatments in line with clinical guidelines
Monitoring patient progress and escalating concerns appropriately
Maintaining accurate clinical records
Working autonomously while liaising with GPs and wider healthcare teams
Essential Requirements
Registered Nurse (NMC registered)
Proven experience working in a community nursing role
Full UK driving licence and access to own vehicle
Strong communication and organisational skills
Right to work in the UK
