As a Community Nurse, you will be responsible for delivering high-quality, patient-centred care within the community setting. You will work independently and as part of a multidisciplinary team to support patients in their own homes, helping to promote recovery, independence, and wellbeing.

Key Responsibilities:

• Deliver nursing care to patients in the community

• Undertake clinical assessments and care planning

• Administer medications and wound care

• Maintain accurate patient records

• Work collaboratively with other healthcare professionals

Requirements:

• NMC registered Nurse

• Previous community nursing experience is essential

• Strong communication and organisational skills

• Ability to work independently
