You will provide high-quality nursing care to patients within their homes and local community settings, supporting both short- and long-term care needs. The role requires autonomy, professionalism, and a compassionate approach to patient care.

Key Responsibilities:

• Deliver safe and effective nursing care in the community

• Carry out patient assessments and implement care plans

• Provide wound care, medication administration, and health education

• Liaise with GPs, hospitals, and community teams

• Maintain accurate documentation

Requirements:

• Valid NMC registration

• Proven community nursing experience required

• Excellent clinical and communication skills

• Ability to manage workload independently
