Hours:

• 08:00 – 20:00 or

• 14:00 – 22:00

In this role, you will deliver essential nursing care to patients within the community, supporting clinical outcomes while promoting patient independence. You will be required to travel between patient visits, making a driving licence essential.

Key Responsibilities:

• Provide community-based nursing care

• Conduct patient assessments and ongoing reviews

• Deliver wound care, medication administration, and clinical interventions

• Travel between patient homes across Hertfordshire

• Communicate effectively with multidisciplinary teams

Requirements:

• NMC registered Nurse

• Must be a car driver

• Previous community nursing experience preferred

• Strong time management and communication skills
